04 · Session notes
What happened in the session, in your own words.
120 session notes, two per page, written in order. Each one has the client code, session number, date, and format (in person, telehealth, phone, or no-show), then the note and the plan for the next session.
They’re for the sessions you want to think through again or bring to supervision. Your clinical record stays where your practice keeps it.

05 & 07 · Supervision and CE
Supervision hours and CE, where you can see them.
If you’re working toward licensure, log every meeting: 32 supervision records, each with what you bring in and the feedback you take away.
Then total your hours by month, with individual and group supervision, client hours, and supervisor initials, and keep hours required, completed, and remaining on the same page. CE goals, license renewal, and a CE log sit in the same book.

So each client gets a code, from C-01 to C-40, and the rest of the book, including session notes and supervision, uses only that code. The book suggests keeping the key that matches codes to clients somewhere else, such as your EHR.

Does it have dates?
No. You write in the month, the year, and the dates, so you can start in any month and lose no pages. Weeks run Monday to Sunday.
Are times printed on the weekly pages?
No. Each day has a narrow column on the left for times. It is left blank because every practice keeps its own hours.
Is it for counselors, LMFTs, and social workers?
Yes. It was made for therapists, counselors, social workers, and psychologists in the US, in private practice or working toward licensure.
Does it replace my EHR?
No. As the book itself says, it doesn’t replace your clinical records or the recordkeeping requirements of your state and licensing board. It’s a planner and a notebook.
Can I track supervision hours for my license?
